If you do not have insurance, payment is made directly to 14th Street Chiropractic.  We gladly accept checks, cash, Visa, and Mastercard.  Flexible payment plans are available.  If you have health insurance that we do not participate in, it is always best to call your insurance carrier to know the specific benefits of your plan.  It is the patients' responsibility to meet their deductibles.  Once deductibles are met, they will be responsible for any co-insurance amount at the end of each visit.

Also, 14th Street Chiropractic will provide you with a computer generated form that is properly coded with procedures and your appropriate diagnosis which you can then submit for reimbursement to your carrier.  If your health insurance company requires preauthorization, an initial report, or a treatment plan, we will provide that also.  14th Street Chiropractic is ready to assist you in any way at all times. 

Health Savings Accounts

Pay for your chiropractic treatment with pre-tax dollars and save.  

If your employer sponsors a health care spending program in which you participate, ask us for a receipt for any payments that you make.  It is certainly worth an inquiry to find out if your company sponsors a program.
